Much of the information you see on web sites tells all of the good about certain dental services, and none of the negative side. When you want to offer enough information to get true informed consent, you have to tell a fair and balanced story. That’s what we do on our Informed Consent forms. In my opinion they sometimes emphasize the negative a little too much, but you might want to be aware of any possible negatives associated with a particular treatment. Sometimes patients want general information about the work we do, and sometimes they want all of the details. If you want to know a lot of details about some of the services we offer, you can get that information by studying our informed consent sheets. They are available here as PDF files, which you can either read online or download and print as you prefer.

Informed consent forms are available for the following procedures:

Root Canal Therapy
Sedation Dentistry
Dental Implants
Crowns, Veneers, and Bridges
Invisalign® Orthodontics
